To understand the effectiveness of the system in counteracting the readmission at the hospital after 30-days, we will include 210 older patients aged 65 years or older admitted to the geriatric ward in 3 pilot sites over at least 3-months trial.
The older participants will be recruited in the different pilot sites
Italy (INRCA)
100 older patients (50 intervention vs. 50 control) and their respective informal caregivers.
Norway (Farsund municipality)
80 older patients (40 intervention vs. 40 control) and their respective informal caregivers.
Romania (HINS)
30 older patients (15 intervention vs. 15 control) and theirrespective informal caregivers.
